Lack of attention to detail and failure to follow instructions
“The lawyer was agitated and became defensive. He did not write down any of the information that the judge told him, even though that made the judge more irritated. He then tried to fix the problem as he remembered it, even though the judge had given him extremely detailed instructions of exactly what the new document needed to say, he submitted again and was again denied because he had not followed the judge's very explicit directions.”
