There was a nice lady at the front desk who came to my aid, explaining which forms I needed to fill out suggesting that I make copies first so that no mistakes will be made on the originals. She was right!

Great law firm they will get you most money from accident and fights to lower medical bills. Answer questions with in hour. Hopefully I will not need them again but if happens they ones to call right away. High recommended them.
