Doing my case, I asked for a form 16 because, I knew there would be further problems with my back and, I was right. I got hurt 2015 an, I later had back surgery. It's 2019 still having difficulty an, I half to use a cane an back brace the rest of my life. My case was closed by Carter marling law firm without him doing the form 16 very sad situation.
