Over a year on a know brauner milking me for money

Twice now no call no showed on me!! Waste of time. The paper-work was never mentioned to me the first time. So, I kindly gave them another chance and waited for the paperwork. (send it way before the night before) Just a suggestion and MENTION IT!! How can I look for an e-mail that was never explained for me to do. I tried calling two days before on the second appointment to see where the paperwork was with no response. I received the paperwork at 4pm the day before the consult. Keep in mind its over five pages wanting all kinds of Info, I had it almost all done and was going to send it during my consult but, they could not even bother calling me to see if I had completed it or anything. Just straight NO-CALL NO-SHOW...
