If you want additional copies of your records in any format, please provide me with a HIPAA compliant release and let me know in what format you want those records and DHHA will provide them.
I did not provide him with the form he requested and he still accessed my private information. He's is currently being employed by Denver Health as a defense attorney. YOU CANT DO THAT PAUL!
